Patient Discharge ‘Time Out’ Reduces Medication Errors

June 27, 2015 – When a patient is ready to be discharged, a nurse reviews the medications that the patient is currently taking and will need to take after he or she leaves. Ideally, this medication reconciliation process will go smoothly–and there will be no discrepancies. Of course, in the real world, discrepancies are often detected during this process, and some aren’t picked up at all.
