If you are expecting to find a quiet sewing circle at the North Preston Seniors’ Club, the burst of colour and energy that hit you when you walk through the doors is enough stop you in your tracks. The conversation is nothing short of boisterous as a dozen or so sets of nimble fingers dance across fabric and wool. Among them is an honorary member: nurse practitioner Carolyn Mitchell.
The growing pile of tiny purple knit hats will find a home atop newborns at the IWK soon. It was Carolyn’s idea. Ivory Provo is the president of the North Preston Community Health and Wellness Centre’s Senior Citizens’ Club and remembers how it started. “She came in to our seniors room one day and she was sitting down talking to me and I was knitting, and she came up with this idea that we should do it for the children’s hospital.”
“She just comes in and talks with us.” says Norma Downey, another member of the club who has a growing appreciation for what Carolyn brings to the group. “She’s really nice. Friendly. Any questions that you ask her she’ll find the answers.”
Carolyn leverages the relationship to get a better understanding of what the health needs are in the community she serves. “A simple knitting project has built up a relationship with the seniors. They feel comfortable around me and I can identify some needs that they have.”
It’s this type of unusual interaction from a primary health care provider that is becoming more familiar in Nova Scotia, but for many it’s still something new “A nurse practitioner is a nurse who has advanced education and training that allows us to prescribe medications, to order tests, to diagnose certain conditions and to manage chronic health conditions so we do a lot of the same things that a family doctor does,” says Carolyn, “…but we’re still nurses so we still come to the patient encounter from a nursing perspective….You could be seeing patients in the office then running out to do a home visit for someone who can’t make it to the office, or then meeting group in the evening but having the freedom to be able to meet the needs of the community, and meet them where they are.”
A few days later, the group is assembled in a much more formal setting in an IWK boardroom in Halifax, but the energy level is just as high as it was back in North Preston. The group is enjoying a small thank you tea from the IWK before heading off for a tour. Carolyn is there too, sharing in the satisfaction of donating boxes of handmade hats and blankets that will surely go to good use.
Just as important for Carolyn is the satisfaction of knowing that behind every stitch in the impressive pile there was conversation. Sometimes they talked about what they’e eating, how they’re struggling with mobility, or just musing about how much candy is too much before bedtime for their grandkids. Carolyn has woven herself inside of those conversations and the hours of chatting about the things that colour their lives, quietly building a better understanding of how to deliver the healthcare they need. “I spend some part of my day cutting blankets and knitting, and I don’t think a lot of places would allow me to do that as a nurse practitioner but it’s something that invests in building the relationships with the community members, and we build a trust.

Open Visitation: New ICU Visiting Hours Policy Puts Patients First

The patient-centered, open visitation policy implemented at the
Intermountain Medical Center in Murray, Utah, puts families first, said
Lorie Mitchell, RN, MSN, nurse manager at the hospital’s 24-bed
Shock/Trauma ICU. The policy is designed to provide unlimited visitor
access while maintaining good patient-centered care, she explained. “As a
result, patient and family satisfaction has improved, family
involvement has grown, and nursing support of policy implementation has
increased.”

What Makes Nursing Work?


Mental Health Services in School-Based Health Centers: Systematic Review

Mental health issues affect 20–25% of children and adolescents, of which few receive services. School-based health centers (SBHCs) provide access to mental health services to children and adolescents within their schools. A systematic review of literature was undertaken to review evidence on the effectiveness of delivery of mental health services in SBHCs. Databases were searched extensively for research studies published between January 1990 and March 2014. Data analysis was based on the method proposed by the Centre for Reviews and Dissemination. Twenty-three studies were selected for review. Each study was explored for accessibility and content of mental health services in SBHCs. SBHCs provide access and eliminate barriers to mental health services. Students who exhibited high-risk behaviors were more likely to have sought services at the SBHC. However, there is a lack of high-quality research evaluating mental health services in the SBHCs and their effect on children and adolescents.


A Systematic Review of Consent Procedures, Participation Rates, and Main Findings of Health-Related Research in Alternative High Schools From 2010 to 2015

There is a well-established link between educational attainment and health. Alternative high schools (AHSs) serve students who are at risk for school dropout. Health-related research conducted in AHSs has been sparse. Achieving high participation rates is critical to producing generalizable results and can be challenging in research with adolescents for reasons such as using active consent. These challenges become greater when working with vulnerable populations of adolescents. In this systematic review, we examined health-related studies conducted in AHSs between 2010 and 2015. Results indicated that (1) health-related research in AHSs has increased over the past 5 years, (2) AHS students continue to experience significant disparities, (3) active consent is commonly used with AHS students, (4) 42% of studies reported participation rates or provided enough information to calculate participation rates, and (5) school nurses are missing from health-related research conducted in AHSs. Implications for future research and school nursing are discussed.


A Systematic Review of Literature on Culturally Adapted Obesity Prevention Interventions for African American Youth

Obesity and overweight prevalence in African American (AA) youth continues to be one of the highest of all major ethnic groups, which has led researchers to pursue culturally based approaches as a means to improve obesity prevention interventions. The purpose of this systematic review was to evaluate culturally adapted obesity prevention interventions targeting AA youth. A search of electronic databases, limited to multicomponent culturally adapted obesity prevention controlled trials from 2003 to 2013, was conducted for key terms. Eleven studies met inclusion criteria. We used the PEN-3 model to evaluate the strengths and weaknesses of interventions as well as to identify cultural adaptation strategies. The PEN-3 model highlighted the value of designing joint parent–youth interventions, building a relationship between AA mentors and youth, and emphasizing healthful activities that the youth preferred. The PEN-3 model shows promise as an overarching framework to develop culturally adapted obesity interventions.


Addressing Parental Vaccination Questions in the School Setting: An Integrative Literature Review

School nurses work in a unique environment with key opportunities to address parental concerns and questions regarding their child’s health. A common concern for parents during school enrollment is childhood vaccination safety and efficacy. As public health leaders, school nurses are well respected among parents, therefore school nurses are in a prime position to educate parents and promote childhood vaccinations while also dispelling common vaccination myths. The purpose of this integrative literature review is to synthesize evidence-based answers to common parental questions regarding childhood vaccinations.


School Nurses Experiences of Managing Young People With Mental Health Problems: A Scoping Review

Prevalence of mental health disorder is increasing among young people. It is recognized that early intervention is essential in supporting young people, and care provided within schools to support emotional well-being is recommended as part of this process. A scoping review was undertaken examining school nurses’ experiences of supporting the mental health of schoolchildren. Findings suggest that school nurses have a central role in supporting young peoples’ mental health, although a number of barriers exist which impact on school nurses’ preparedness for this aspect of their role. While there are inconsistencies in the provision of mental health training available to support school nurses, when training is implemented it has positive outcomes for service provision and quality of care. A number of challenges for school nurses were also identified, and recommendations for practice are suggested in the article.
